29 CFR 1910.23(c)(3): Regardless of height, open-sided floors, walkways, platforms, or runways above or adjacent to dangerous equipment, pickling or galvanizing tanks, degreasing units, and similar hazards were not guarded with a standard railing and toe board:    a)  Employees were using Alpine Linear Saw (ALS) HiSpeed Model 276 saws with waste conveyor belts which deposit wood debris into unguarded floor openings where a screw conveyor is located, on or about 05/18/16.    b) Employees were working near the maintenance shop and/or moving cut wood pieces to the truss assembly area near the Patz Waste Conveyor where there was an inadequately guarded floor opening where a screw conveyor is located (the railings were not the required height and there were no toe boards), on or about 05/18/16.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) On May 10, 2016, while retrieving a small wood piece, an employee received a partial amputation of their left ring finger from unguarded rotating parts powering an Alpine Linear Saw (ALS) conveyor belt.
