29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:    (a)  City Market, at 250 East Valley Road, El Jebel, CO 81623:  On and before May 18, 2016, the employer did not ensure that employees used personal fall arrest systems, with established anchorage points, when performing maintenance on elevated work platforms.  A maintenance employee was servicing an air handler above the meat preparation area, and was exposed to fall hazards when accessing and working in this area.  No alternative means of fall protection was in use.  Condition exposed maintenance employees to fall hazards of approximately 11 feet as they serviced equipment located on top of the units.
