29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered.      On or about June 7, 2016:  An employee working in the Geomet lab area received chemical burns to his forearms while performing the QA process of cooking off the coating of small parts.  The employee was exposed to chemicals such as but not limited to sodium hydroxide. The employer did not provide personal protective equipment such as but not limited to protective sleeves to prevent chemical burns to the employee's arms.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use.      On or about May 18, 2016:  Employees working in the Geomet lab area are exposed to corrosive chemicals such as but not limited to sodium hydroxide and boric acid.  The employer did not provide an emergency eyewash station in or near the area.

On or about May 18, 2016:  Employees working in the Geomet lab area are exposed to fire and smoke hazards.  The employer did not ensure that the fire extinguisher was mounted, identified, and positioned so as not to cause injury to the employees.

The employees were not provided with proper information on the respirators to include but not limited to the limitations of the respirators.
