29 CFR 1910.147(c)(4)(ii)(B): The energy control procedure did not clearly and specifically outline procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: On or about April 28, 2016, in the printing area where employees were exposed to struck-by hazards while performing servicing and/or maintenances activities such as changing out the cutting blade on the Champion cutter machine, without having specific procedures.
