Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment
and a place of employment which were free from recognized hazards that
were causing or likely to cause
death or serious physical harm to employees in that employees were exposed
to severe burn injuries from the
potential dust explosion that could have resulted from, or have been
exacerbated by, recycling the air back
into the building without a high speed abort gate, a lack of a back flash
damper and the lack of deflagration
vents for the dust collector:
(a)Standard Register Company - The dust collection system was used to
collect paper dust.  The
dust collection system, duct work leading to the dust collector and the
recycled air were not
protected by a deflagration vent,  a back flash damper and a high speed
abort gate system in
the event of a fire or explosion, on or about April 1, 2011.
ABATEMENT NOTE: Among other methods, one feasible and acceptable abatement
method to
correct this hazard includes following the National Fire Protection
Association
(NFPA) 654, Standard
for the Prevention of Fire and Dust Explosions from the Manufacturing,
Processing, and Handling of
Combustible Particulate Solids, 2006 Edition, including, but not limited
to installing a high speed abort
system on the return air duct work, installing a back flash damper on the
inlet side, a deflagration
system or a suppression system on the dust collector and/or duct work.
All proposed control measures shall be approved for each particular use by
a competent engineer or
other technically qualified person.  Ninety (90) day progress reports are
required during
the abatement period.
*STEP 1
Submit to the Area Director a written, detailed plan of abatement
outlining a schedule for the
implementation of engineering and/or administrative measures to control
employee exposure to
hazardous substances as referenced in this citation.
This plan shall include, at a minimum, target dates for the following
actions which must be consistent
with the abatement dates required by this citation:
1.Evaluation of engineering/administrative control options.
2.Selection of optimum control methods and completion of design.
3.Procurement, installation and operation of selected control measures;
and
4.Acceptance or modification/redesign of controls.
*STEP 2
Abatement shall have been completed by the implementation of feasible
engineering upon verification
of their effectiveness in achieving compliance.

Abatement certification required within 10 days after abatement date. The
certification shall
include a statement that abatement is complete, date and method of
abatement, and states
employees and their representatives were informed of this abatement.
