29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   At or about May 20, 2016 and times prior thereto, at the worksite locate at or near 424 Industrial Park Road in Heber Springs, Arkansas, the employer did not guard the used portion of the blades on the bandsaws in the following instances:  Instance A: Employees using the bandsaw in the metal integrity department did not fully guard the non-working portion of the blade. Employees use the bandsaw to cut aluminum parts to be tested for elasticity.  Instance B: Employees using the bandsaw in the degate department did not fully guard the non-working portion of the blade. Employees use the bandsaw to separate aluminum parts.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  At or about May 20, 2016 and times prior thereto, at the worksite located at or near 424 Industrial Park Road in Heber Springs in Arkansas, the employer did not conduct annual inspections of the Lock-out/Tag-out procedures. This condition exposed employees to unexpected start-up hazards.

29 CFR 1910.1030(c)(1)(iv): The Exposure Control Plan shall be reviewed and updated at least annually and whenever necessary to reflect new or modified tasks and procedures which affect occupational exposure and to reflect new or revised employee positions with occupational exposure. The review and update of such plans shall also:  At or about May 20, 2016 and times prior thereto, at the worksite located at or near 424 Industrial Park Road in Heber Springs, Arkansas, the employer had not done an annual review of the bloodborne pathogen program. This condition exposed employees to biological hazards.
