There were no written procedures on how to do this task safely and on 05/10/2016, maintenance management failed to shut off the conveyor's "off" rollers and de-energize the conveyor drive motor they were working on prior to commencing maintenance on belt drives. This resulted in the amputation of the night shift lead's finger.   b) For the facility at 3900 Ave. G NW, Winter Haven, FL 33880, as observed on or about 05/23/2016 and 06/16/2016, authorized employees were not trained on lockout tagout procedures for work on the belt that ran under auto web 4 (AW4).
