29 CFR 1910.23(a)(8): Every floor hole into which persons can accidentally walk were not guarded:  a) Basement - On or about May 23, 2016, employees were exposed to caught-in and tripping hazards due to a floor hole approximately measuring 8" in length by 10" in width by 17" in depth.    b) Basement - On or about May 23, 2016, employees were exposed to caught-in and tripping hazards due to a floor hole approximately measuring 5" in length by 7" in width by 17" in depth.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  a) Basement - On or about May 23, 2016, a relocatable power tap (RPT) or power strip powering various equipment was not installed and used in accordance with instruction in the listing, labeling, or certification such as, but not limited to, daisy chaining two RPTs together that powered the various equipment.  The various equipment consisted of equipment such as, but not limited to, a microwave and toaster oven.     b) Basement - On or about May 23, 2016, the employer was using a relocatable power tap (RPT) or power strip which is not designed to for industrial use to power a microwave and toaster oven.  c) Basement - On or about May 23, 2016,  a relocatable power tap (RPT) or power strip powering various electrical equipment along with a permanent extension cord plugged into the RPT.  The permanent extension cord plugged into the RPT was then run through the ceiling over a doorway to power flood lights.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:  a) Basement - On or about May 23, 2016,  a relocatable power tap (RPT) or power strip powering various electrical equipment along with a permanent extension cord plugged into the RPT.  The permanent extension cord plugged into the RPT was then run through the ceiling over a doorway to power flood lights.  The permanent extension cord was used as permanent wiring.  b) Basement - On or about May 23, 2016, flood lights with flexible cords were used as permanent wiring and lighting throughout the basement.

The flexible cord on the flood lights was plugged into the permanent extension cord that was run through the ceiling over a doorway that was plugged into a relocatable power tap (RPT).
