29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects were not protected by protective helmets: a) On or about 05/23/16, on the ground on the south side of a house at 802 Niagara St., Buffalo, NY: Employee walked in an area on the ground below the roof where roof tear-off work was being done. Debris was falling and being thrown off the roof. Employee(s) were not wearing head protection. ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toe boards, safety net systems, or personal fall arrest systems: a) On or about 05/23/16, on the south side of the roof of a house at 802 Niagara St., Buffalo, NY: Three employees engaged in roofing tear-off work on a steep roof were exposed to a fall of approx. 20 foot. The employees were wearing full body harnesses, but were not tied off to a lifeline. No other fall protection system was utilized. Pitch of the roof was 12 in 12. NO ABATEMENT DOCUMENTATION REQUIRED
