29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:    a) On or about 5/6/19, on a roofing jobsite, at a house located at 1133 Hertel Ave., Buffalo, NY:  Ladder Jack Scaffolding had been erected on the east side of the house. Employees were on the scaffolding installing the gutters, and were exposed to a fall of approx. 18 ft. because fall protection was not being utilized. The roof anchors & lifelines that were in place on the roof had been removed and the roof cap was installed prematurely while other employees were left working on the ladder jack scaffold unprotected.     NO ABATEMENT CERTIFICATION REQUIRED

The employees were exposed to a fall of approx. 20 ft. because fall protection was not being utilized. The roof anchors & lifelines that were in place on the roof had been removed and the roof cap was installed prematurely while other employees were left doing roofing work unprotected. Pitch of the roof was approx. 8 in 12.      NO ABATEMENT DOCUMENTATION REQUIRED    Green Earth Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1369157, citation number 1, item number 2, and was affirmed as a final order on 3/4/2019, with respect to a workplace located at 3125 Main Street, Buffalo, NY 14214.
