29 CFR 1926.416(a)(1): The employer permitted an employee to work in such proximity to any part of an electric power circuit that the employee could contact the electric power circuit in the course of work, without the employee being protected against electric shock by deenergizing the circuit and grounding it or by guarding it effectively by insulation or other means. This violation occurred on or about May 24, 2016, and at times prior thereto, at 7930 NW Expressway, Oklahoma City and other stores throughout Oklahoma City, the employer did not ensure employees, required to remove neon lighting equipment and install LED lighting equipment, deenergize the electrical energy sources exposing employees to the hazards of coming into contact with energized equipment.

29 CFR 1926.1203(a): Before it begins work at a worksite, each employer did not ensure that a competent person identifies all confined spaces in which one or more of the employees it directs may work, and identifies each space that is a permit space, through consideration and evaluation of the elements of that space, including testing as necessary. This violation occurred on or about May 24, 2016, and at times prior to, at 7930 NW Expressway, Oklahoma City, General Lighting & Sign Service, Inc. did not have a competent person identify the overhead crawl spaces as confined spaces or consider and evaluate the crawls spaces as possible permit-required confined spaces.

29 CFR 1926.1207(a): The employer did not provide training to each employee whose work is regulated by this standard, at no cost to the employee, and ensure that the employee possesses the understanding, knowledge, and skills necessary for the safe performance of the duties assigned under this standard. This training must result in an understanding of the hazards in the permit space and the methods used to isolate, control or in other ways protect employees from these hazards, and for those employees not authorized to perform entry rescues, in the dangers of attempting such rescue. This violation occurred on or about May 24, 2016, and at times prior to, at 7930 NW Expressway, Oklahoma City, General Lighting & Sign Service, Inc employees required to work in a permit-required confined space were not provided with training to safely perform live electrical work exposing employees to the hazards of coming into contact with live electrical conductors.
