29 CFR 1926.54(d): Standard laser warning placards were not posted in areas in which lasers were used: A.) Frontier General Contracting, Inc., 2088 Interchange Road, Unit #0150, Erie, PA 16565, on or about May 25, 2016 and times prior thereto: Employee from a sub-contractor were utilizing a Pacific Laser System HVR 505G rotary laser while performing work activities including, but not limited to, installing drop ceiling grid work. Standard laser warning placards were not posted in the area in which the rotary laser was being used.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: A.) Frontier General Contracting, Inc., 2088 Interchange Road, Unit #0150, Erie, PA 16565, on or about May 25, 2016 and times prior thereto: Employees were installing a plywood overhang on the front of the store. Employees were utilizing a personal fall arrest harness and 6-foot shock absorbing lanyard when working at a height of 10.66-feet from ground level. In the event of a fall, the employee would fall 10.66-feet though the overhang grid work / openings in the grid work, or 14.47-feet over the overhang. The fall protection system was not designed and in that the employee would contact the ground prior to the PFAS completely stopping the fall.
