(b) Shop Floor, Opposite Office: On or about May 23, 2016 and times prior, the employer did not ensure that machine guarding was provided on a DoAll Horizontal Band Saw (Model C-1216A) to protect employees from the unused portion of the blade. (c) Shop Floor, Outside Office: On or about May 26, 2016 and times prior, the employer did not ensure that adequate machine guarding was provided on a Delta Drill Press (ID: 2007 31-XL) to protect employees from the rotating chuck and the unused portion of the bit.
