(a) South Charleston, WV:  On or about May 27, 2016 and times prior, an employee using a Bostitch pneumatic nail gun to secure furring strips to a roof prior to applying metal panels was wearing sunglasses that did not offer protection against flying fragments from nails or other materials.

29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupters for personal protection.    (a) South Charleston, WV:  On or about May 27, 2016 and times prior, employees were using a Porter Cable 7 1/4 inch, 15 amp circular saw to cut furring strips without a ground fault circuit interrupter.  Power to the circular saw was being provided by a 2 prong, 100ft. extension cord that was plugged into an internal outlet of the residential home of which was not equipped with a ground fault circuit interrupter.

(a) South Charleston, WV:  On or about May 27, 2016 and times prior, employees were exposed to falls of 11 foot to the uneven ground while applying furring strips and metal panels to a 6/12 pitched residential roof without the use of a fall protection system.

(a) South Charleston, WV:  On or about May 27, 2016 and times prior, employees required to prepare a 6:12 pitched roof for the installation of a metal roof were exposed to falls up to 11 foot to the ground and were not trained on the hazards of working from heights and/or the procedures to follow to protect themselves.
