29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) Front of house: On or about November 8, 2017, employees were exposed to a fall of up to 10 feet 5 inches while using a 25 foot Werner extension ladder to access the roof to remove and install shingles. The ladder was not extended 3 feet above the roof edge.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501: a) 425 Allen Dr: On or about November 9, 2017 and times prior, two employees were observed removing and replacing shingles from the roof of a one story residential home. Employees were not protected from falling by a guardrail, safety net or personal fall arrest system. The IBC Corporation dba IBC Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.501(b)(13) which was contained in OSHA inspection number 1150932, citation number 1, item number 3a and was affirmed as a final order on 6/30/16, with respect to a workplace located at 340 Second Ave, South Charleston, WV 25303.
