29 CFR 1904.29(b)(1): A log of all recordable work-related injuries and illness (OSHA Form 300 or equivalent) was not completed in the detail required by the regulation:  At the facility - The OSHA 300 log for the year 2016 was not completed in the detail required by this section for the following reasons or instances:      a)    For all nine cases, Where the event occurred, Column E,  was recorded as Pioneer Plastics -  Auburn, not the department or area within the Auburn facility, where the event occurred.      b)    In Column F, the object/substance that directly injured or made the person ill, was not listed. As an example the only entry made in column F for case #2 is crushed finger, left.
