29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: (a) On or about May 24th, 2016 for employees exposed to cave in hazards while installing a 24 water main in an unprotected trench approximately 11 feet deep in the vicinity of 43rd Avenue & Normandy Street in Bismarck, North Dakota. Note: This company was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard which was contained in OSHA Inspection Number 1068403, Citation 1, Item 1, and was affirmed as a final order on October 15th 2015 with respect to a workplace near the intersection of First Street & Tenth Avenue NE, in Mandan North Dakota.
