29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: 82 Deer Park Drive, East Longmeadow, MA On May 31, 2016 and at times prior, employees who cleaned the coater section of the number three treater machine without isolating the energy sources were exposed to hand/arm injuries. On May 31, 2016, an employee received hand and arm injuries while cleaning that coater section .

29 CFR 1910.147(c)(6)(i): The employer had not conducted a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard were being followed. 82 Deer Park Drive, East Longmeadow, MA As of May 25, 2016, the employer had not performed a periodic inspection of the energy control program for more than two years. The employer had not inspected each procedure while in-use to correct any deviations or inadequacies. The employer also had not reviewed each procedure between the inspector and each authorized employee to reinforce each employee's responsibilities under each energy control procedure.
