29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees: On or about May 24, 2016, the employer did not ensure that authorized employees servicing the Johnnie #1 had acquired the requisite knowledge and skills to safely apply and use energy control devices to isolate energy sources prior to commencing servicing activities. Employees were exposed to lacerations, fractures and amputations. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about May 24, 2016, an assembly operator suffered multiple fractures to the right lower and upper arm. The employer did not ensure that employees were not required to perform the following sequence of actions prior to servicing the Johnnie #1: 1) Machine or equipment shutdown. 2) Machine or equipment isolation. 3) Lockout or tagout device application. 4) Verification of isolation. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
