29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels : a) Plant - On or about 06/28/16 and ongoing, employees operating QVL Straight were exposed to continuous noise at 72.4% of the permissible daily noise level of (8-hour time-weighted average sound level of 80 dBA) or equivalent time-weighted of approximately 87.6 dBA. The employee had not been provided with an annual audiogram in 2015. b) Plant - On or about 06/28/16 and ongoing, employees operating QVL Straight were exposed to continuous noise at 62.4% of the permissible daily noise level of (8-hour time-weighted average sound level of 80 dBA) or equivalent time-weighted of approximately 86.6 dBA. The employee had not been provided with an annual audiogram in 2015. NO ABATEMENT CERTIFICATION REQUIRED
