29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift. a) Roadway and concrete sidewalk, 153-155 East Independence Street, Shamokin, PA - Employees replacing siding on a commercial building from the elevated basket of a JLG Articulating Boom Lift Model 450 AJ Series II (Serial No. 0300161565) did not wear fall protection and were exposed to a twenty two-foot (22') fall hazard, as observed on June 2, 2016.
