29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.  On or about 27 May 2016 and at times prior thereto, two employees without head protection were passing materials from the second story floor to another employee working approximately 9 feet above, exposing the two ground level employees to the hazard of being struck by falling materials.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  On or about 27 May 2016, two employees without eye protection were using pneumatic nail guns to perform framing activities on a residence under construction, exposing the employees to the hazard of being struck by flying objects.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  On or about 27 May 2016, two employees without fall protection were performing framing activities on a residence under construction, exposing the employees to the hazard of falling approximately 19 feet, 7 inches to the ground below.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step.  On or about 27 May 2016, two employees performing framing activities used the top step of a portable step ladder to access the top cap of the second story walls of a residence under construction, exposing the employees to the hazard of falling approximately 6 feet to the ground below.
