29 CFR 1910.37(b)(6): Each exit sign must be illuminated to a surface value of at least five foot-candles (54 lux) by a reliable light source and be distinctive in color. Self-luminous or electroluminescent signs that have a minimum luminance surface value of at least .06 foot-lamberts (0.21 cd/m2) are permitted:    On or about 4/25/2016 at 13129 Harland Drive Covington, GA - Employees required to fight incipient level fires were exposed to potential smoke inhalation and thermal burn hazards where emergency exit signs throughout the plant were not illuminated as required at locations including but not limited to:    a.    EXIT marking above the doorway from production area to the front office and break room was made of flat red stickers crossing over the junction box,    b.   EXIT marking in the hallway in production area to the left of the entrance from the front office and break room was made of flat red stickers adhered to a flat piece of aluminum,    c.    EXIT marking in the hallway in production area to the right of the entrance from the front office and break room was made of flat red stickers adhered to a flat piece of aluminum.

29 CFR 1910.147(c)(4)(i): Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  On or about 4/17/2016 at line 3 - an employee suffered the amputation of his left ring, middle, and index fingers while performing a "B" clean of the line where the employer failed to insure that operators were using the specific procedures and not checklists that were developed to support line cleaning which were not containing specific lockout procedures.
