29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system:  a) Building E: On June 2, 2016 an employee performed framing activities on a 4 in 12 pitched roof without a fall protection system in place.  The employee was exposed to a 30 foot fall hazard to the parking lot below.   b) Building E: On June 2, 2016 an employee performing framing activities on a 4 in 12 pitched roof with a personal fall protection system in place, did not have the chest strap or the leg straps of the body harness buckled.  The employee was exposed to a 30 foot fall hazard to the parking lot below.   c) Building E: On June 2, 2016 employees performed framing activities on the third floor.  Complete guardrail systems were not in place at the wall openings for the balconies.  No guardrail system was in place at one opening, while the other was missing a mid-rail.    Note) Certification and documentation of corrective action ARE required for this item.

Defects included, but may not be limited to broken metal spreader bars/locking devices.
