29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about June 3, 2016, employees were exposed to a fall hazard greater than 6 feet in height to unpaved ground level below while installing wood subflooring on the second floor without having fall protection measures in place.

29 CFR 1926.1060(b): The employer did not provide retraining as necessary for each employee using ladders and stairways so that the employee maintained the understanding and knowledge acquired through prior training:  On or about June 3, 2016, employees were exposed to a fall hazards of approximately 6 feet in height while demonstrating a lack of understanding for ladder safety onsite when using an A frame portable ladder to access the first landing of the second floor that was not at least 3 feet above the landing and stepping on the top step.
