29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.   (a) Residential construction site, protective helmets were not used to protect workers from the hazard of falling or flying tools and materials as a result of employees walking and working below workers above them.

(a) Residential home, during roofing operations, at least eight (8) employees, used pneumatic nail guns without the use of protective eyewear.

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502.  (a) Rooftop of a residential home, during roofing operations, employees were not protected from a fall hazard of approximately 8-feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   (a) On the jobsite, employees used a portable ladder which side rails did not extend at least 3-feet above the upper landing surface. Employees were exposed to a fall hazard of 8-feet.
