29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person: On or about June 02, 2016 at 924 West Fifth Street Newton, KS 67114: Employees engaged in service renewal activities, including but not limited to replacing a four-bolt saddle fitting with a one-bolt saddle fitting, were exposed to fire and explosion hazards in that an inspection by a competent person authorized to take prompt corrective measures was not conducted to ensure the rectifier was turned off.
