29 CFR 1926.403(g):Electrical equipment which was used did not have other markings giving voltage, current, wattage, or other ratings as necessary: Employees working at Johnson and Foster Dr., Shawnee, Kansas 66203 engaged in trenching activities were exposed to electrical hazards in that a home-made outlet device which did not have markings giving voltage, wattage, or other ratings was used to power tools on-site. The device was powered by an extension cord.

29 CFR 1926.405(g)(2)(iv):Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: Employees working at Johnson and Foster Dr., Shawnee, Kansas 66203 engaged in trenching activities were exposed to electrical hazards in that the extension cords used to supply power to the home-made outlet device was not equipped with adequate strain relief. As a result the extension cord showed signs of damage. The insulation had pulled away from the plug, exposing the conductors.

29 CFR 1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). Employees working at Johnson and Foster Dr. Shawnee, Kansas 66203 engaged in gas line repair activities, inside of an excavation found to be approximately 7 feet in depth, were exposed to cave-in hazards in that adequate cave-in protective system was not provided.
