29 CFR 1910.178(l)(4)(ii)(D): Refresher training in relevant topics was not provided to the operator when the operator was assigned to drive a different type of vehicle: a) Warehouse- On June 6, 2016, the employer did not ensure that refresher training, an evaluation, and certification of the forklift operators knowledge and skill were performed on relevant topics when the operator was assigned to drive a different type of powered industrial vehicle, such as the Crown stand up forklifts and the Crown PE 4500 series electrical pallet jack.
