29 CFR 1910.178(g)(2): Battery changing and charging installations for industrial trucks were not provided with facilities for flushing and neutralizing spilled electrolyte: The employer is failing to protect employees from chemical hazards associated with adding water to forklift batteries without an available eyewash station. The most recent occurrence of this was evident during the inspection of the facility located at 10320 J Street, Omaha, NE 68127 where the employer has employees adding the water to the batteries in a location that does not have an eyewash readily available. Instances of exposure include but are not limited to: i. The employees are adding water near a sink and the only available eyewash is down two hallways over 100 feet away. ii. The only eyewash station was inoperable at the time of the inspection.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): The employer is failing to train powered industrial truck operators to operate and recognized the struck by hazards associated with operating a forklift as outlined in this subpart. The most recent occurrence of this was evident during the inspection of the facility located at 10320 J Street, Omaha, NE 68127 where the employer has inadequately trained employees operating and maintaining the Drexel model SLT30 Forklift to move product and material throughout the facility. Instances of inadequate training include but are not limited to: i. Employees operating forklifts without having received a formal block of instruction on the safe handling operations of the powered industrial trucks as outlined in the operators manual and confirmed with a written assessment. ii. Employees operating forklifts without having received practical training (demonstrations by the trainer) and practical exercises (performed by the trainee) on the safe handling operations of the powered industrial trucks as outlined in the operators manual. iii. Employees operating forklifts without having been evaluated on their performance in the workplace. Omaha Fixture Manufacturing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(l)(1)(i), which was contained in OSHA inspection number 506038, citation number 1, item number 2 and was affirmed as a final order on November 13, 2012, with respect to a workplace located at 10320 J Street, Omaha, NE 68127
