29 CFR 1915.155(a)(1): The employer did not ensure that affected employees wore protective helmets when at risk from being injured by falling objects.    Dockside: The employer did not ensure that all affected employees were protected from head injuries by wearing protective helmets when exposed to overhead hazards.    On or about May 19, 2016, one of employees sustained head and neck injuries from hit by a crane hook while performing tasks.
