29 CFR 1910.179(j)(2)(iv): Hoist chains, including end connections, were not inspected monthly for excessive wear, twist, distorted links interfering with proper function, or stretch beyond manufacturer's recommendations with a certification record which includes the date of inspection, the signature of the person who performed the inspection and an identifier of the chain which was inspected: Throughout the shop. On or about June 2, 2016, and at times prior thereto, employees were exposed to a struck-by hazard from the gantry crane hoist chains not being inspected and certified monthly.

Near the northwest corner of the main shop. On or about June 2, 2016, and at times prior thereto, an employee was exposed to the unguarded rotating shaft of the Mori Seiki CNC lathe during machine operation.
