29 CFR 1910.179(j)(2)(iv): Hoist chains, including end connections, were not inspected monthly for excessive wear, twist, distorted links interfering with proper function, or stretch beyond manufacturer's recommendations with a certification record which includes the date of inspection, the signature of the person who performed the inspection and an identifier of the chain which was inspected:    Throughout the shop.  On or about June 2, 2016, and at times prior thereto, employees were exposed to a struck-by hazard from the gantry crane hoist chains not being inspected and certified monthly.

29 CFR 1910.179(m)(1):  A thorough inspection of all ropes was not made at least once a month and a certification record which includes the date of inspection, the signature of the person who performed the inspection and an identifier for the ropes which were inspected was not kept on file where readily available to appointed personnel:     Throughout the shop.  On or about June 2, 2016, and at times prior thereto, employees were exposed to a struck-by hazard from the gantry crane running ropes not being inspected and certified monthly.

29 CFR 1910.219(c)(2)(i): All exposed parts of horizontal shafting seven (7) feet or less from floor or working platform, excepting runways used exclusively for oiling, or running adjustments, were not protected by a stationary casing enclosing shafting completely or by a trough enclosing sides and top or sides and bottom of shafting as location requires.    Near the northwest corner of the main shop.  On or about June 2, 2016, and at times prior thereto, an employee was exposed to the unguarded rotating shaft of the Mori Seiki CNC lathe during machine operation.

29 CFR 1910.305(b)(1)(ii):   Unused openings in cabinets, boxes, and fittings were not effectively closed:    The west wall near the bank of CNC machines.  On or about June 2, 2016, employees were exposed to electrical hazards when using breaker switches in a distribution panel box, as two unused openings in the panel box were not covered.
