29 CFR 1910.134(g)(1)(i)(A):  Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function:    (a) General Construction Services Inc, at 2601 S Platte River Drive, Denver, CO: On 6/3/16, the employer did not ensure an employee wearing a tight-fitting facepiece did not have facial hair that came between the sealing surface of the facepiece and the face.  Employees performed Class I asbestos work during the demolition of a power plant.  Employees removed thermal system insulation and other asbestos containing materials.  Employer performed asbestos personal sampling and determined that employee exposure to asbestos fibers exceeded the Permissible Exposure Limit (PEL).  Employees were required to wear 3M model series 6800 full facepiece powered air purifying respirators with high efficiency particulate cartridge.  One employee had significant facial hair between the facepiece and the face. This condition exposed the employee to a respiratory hazard.

29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:       (a) General Construction Services Inc, at 2601 S Platte River Drive, Denver, CO: On 6/2/16, the employer did not ensure that listed, labeled, or certified equipment was installed and used in accordance with instructions included in the listing, labeling, or certification.  A temporary power distribution center, aka "turtle", was used to supply power for an asbestos abatement containment area.  An extension cord that terminated with three receptacles was plugged into the "turtle".  A power strip was plugged into one of the receptacles of the extension cord.  The power strip was used for charging powered air purifying respirator batteries and for charging cell phone batteries.  The power strip was not used in accordance with its UL listing.  This condition exposed employees to an electrical hazard.
