29 CFR 1910.134(d)(1)(ii): The employer did not ensure that respirators were used in compliance with the conditions of its certification:    (a)  General Construction Services Inc, at 2601 S Platte River Drive, Denver, CO: On or about 5/16/16 and preceding, General Construction Services Inc did not ensure that respirators were used in compliance with the conditions of its certification.  The employer performed demolition of structures contaminated with lead or asbestos at the Xcel Energy Arapahoe power plant.  Plasma torch cutting crews were required to wear 3M powered air purifying respirators (PAPR) with MSA acid gas/P100 respirator cartridges.  The MSA cartridges were connected to the 3M PAPR's through use of an MSA "t-adapter".  This condition exposed employees to a respiratory hazard.

29 CFR 1926.56(a): While work was in progress, construction areas, ramps, runways, corridors, offices, shops and storage areas were not lighted to the minimum illumination intensities listed in table D-3 of this subpart:     (a) General Construction Services Inc, at 2601 S Platte River Drive, Denver, CO: On 7/14/16 and preceding, General Construction Services Inc did not ensure that while work was in progress, construction areas, ramps, runways, corridors, offices, shops and storage areas were lighted to the minimum illumination intensities listed in table D-3 of this subpart.  The employer performed demolition of structures contaminated with lead or asbestos at the Xcel Energy Arapahoe power plant.  Employees accessed containment areas via stairwells or aisles.  Stairwells and aisles were not provided with at least five foot candles of illumination.  This condition exposed employees to tripping hazards.  Locations lit with less than five foot candles of illumination included but were not limited to the following:    1.  Administrative Building hallway;  2.  Stairwell to access upper levels of Units 3 and 4;  3.  Stairwell used to access conveyor for Units 1 and 2; and  4.  Penthouse area adjacent to containment area for conveyor for Units 1 and 2.

The employer performed demolition of structures contaminated with lead or asbestos at the Xcel Energy Arapahoe power plant.  Plasma torch cutting crews were exposed to lead in excess of its Action Level.  The employer did not ensure that all plasma torch cutting crew members received initial medical surveillance.  This condition exposed an employee to a lead hazard.

The employer performed demolition of structures contaminated with lead or asbestos at the Xcel Energy Arapahoe power plant.  Plasma torch cutting crews were exposed to lead in excess of its Action Level.  The employer did not ensure that all plasma torch cutting crew members who were or may have been exposed at or above the action level participated in a medical surveillance program.  This condition exposed employees to a lead hazard.

29 CFR 1926.102(a)(5): The employer did not ensure that Table E-1 was used as a guide in the selection of face and eye protection for the hazards and operations noted:    (a)  General Construction Services Inc, at 2601 S Platte River Drive, Denver, CO: On 7/14/16 and preceding, General Construction Services Inc did not ensure that Table E-1 was used as a guide in the selection of face and eye protection for plasma torch cutting of metal.  The employer performed demolition of structures contaminated with lead or asbestos at the Xcel Energy Arapahoe power plant.  Plasma torch cutting crews were required to wear 3M model 6886/7986 tinted lens covers over the respirator facepiece lens.  The 3M tinted lens covers did not have an appropriate filter lens shade number for plasma torch cutting.  This condition exposed employees to a radiant energy hazard.
