29 CFR 1926.451(c)(2)(v): The employer used forklifts to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied: a) At the jobsite, on or about June 7, 2016, two employees performing steel erection activity were elevated and working from an unapproved work platform that was supported by a GEHL Telescopic Rough Terrain forklift Model: RS6-34, S/N: RS634DH22911. The unapproved work platform was not secured to the forks or carriage and was moved horizontally to reposition the employee, which exposed the employee to fall hazards up to 16 feet onto the lower concrete and dirt surface.
