(a) New entrance area: On or about April 23, 2024, the employer exposed employees to a slip/fall hazard, in that employees climbed onto and off the mid-rails and top rails of scissor lift platforms to access the roof approximately 4 ft above the platform, with a fall distance to ground of up to 21'7" at the apex.

ARC Industries, LLC was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 1926.453(b)(2)(v), which was contained in OSHA inspection number 1643471, citation number 1, item number 1 and was affirmed as a final order on May 17, 2023, with respect to a workplace located at 100 Addie Exley Rd, Rincon, GA 31326.
