29 CFR  1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.   (a)  New entrance area: On or about April 23, 2024, the employer exposed employees to a slip/fall hazard, in that employees climbed onto and off the mid-rails and top rails of scissor lift platforms to access the roof approximately 4 ft above the platform, with a fall distance to ground of up to 21'7" at the apex.

29 CFR  1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift.  (a)  New entrance area: On or about April 23, 2024, the employer exposed employees to an ejection/fall hazard, in that a harness with a lanyard attached to the boom or basket was not worn by each employee that worked from inside an aerial lift elevated up to 13'6" above the ground.   ARC Industries, LLC was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 1926.453(b)(2)(v), which was contained in OSHA inspection number 1643471, citation number 1, item number 1 and was affirmed as a final order on May 17, 2023, with respect to a workplace located at 100 Addie Exley Rd, Rincon, GA 31326.
