29 CFR 1910.212(a)(1): One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Instances including, but not limited to the following: a) Job site, 65 Washington Street, Pittsburgh, PA: On or about June 2, 2016, during parts production activities on the Leiber lathe unit located in building number 2, machine number 1773, the employer did not ensure that machine guarding was provided over rotating parts, such the chuck head or a collar and bolt balancing systems, exposing employees to caught in/on type machinery hazards. b) Job site, 65 Washington Street, Pittsburgh, PA: On or about June 2, 2016, during parts production activities on the Tos Trencin/Gem 2 lathe unit located in building number 2, machine number 463300910341, the employer did not ensure that machine guarding was provided over rotating parts, such the chuck head or a collar and bolt balancing systems, exposing employees to caught in/on type machinery hazards. c) Job site, 65 Washington Street, Pittsburgh, PA: On or about June 17, 2016, during parts production activities on the Tos lathe unit located in building number 1, identifying number SUS 63 H, the employer did not ensure that machine guarding was provided over rotating parts, such the chuck head, exposing employees to caught in/on type machinery hazards.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed: a) Job site, 65 Washington Street, Donora, PA: On or about July 5, 2016, the employer did not ensure that an annual audit of their energy control procedures were conducted to ensure that the procedures and the requirements of the standard were being followed. Concerns noted during the inspection included but were not limited to, not providing appropriate energy control training, not ensuring that energy control procedures were specific to the machinery they were documented for and allowing assistants to work under the maintenance person's lock.
