29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.  (a) (LOCATION) (IDENTIFY SPECIFIC OPERATION(S) AND/OR CONDITIONS)  a) 3474 Village Park Way, Kennesaw, GA: On or about June 6, 2016, employees accessing a roof were exposed to lower level fall hazards of over 25-feet, in that; the 40-foot ladder being used did not have enough length to allow the side rails to extend at least 3-feet above the landing surface without exceeding its reach capability of 37-feet.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  a) 3474 Village Park Way, Kennesaw, GA: On or about June 6, 2016, employees installing roofing shingles were exposed to eye injuries while operating Hitachi NV 45AB2 coil nailers,  in that; no eye protection was in use.  Juana Mejia Perez dba Mejia Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1061492, citation  number 1, item number 1, and was affirmed as a final order on 10/15/2015, with respect to a workplace located at 370 Rose Garden Lane, Alpharetta, GA 30009.
