29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about June 7, 2016, and at times prior thereto, employees were allowed to operate machines that were found not properly guarded, including but not limited to the following instances: 1) In receiving and finish warehouse areas, trash compactors were operated without a dead-man switch or a guard with interlock device provided. 2) In the finished product warehouse palletizer area, palletizer number 6 had an exposed nip point. 3) In the medium and heavy shop areas, lathe machines were operated without chuck guard provided. 4) In the medium and heavy shop areas, drill presses were used without guard provided. Employees were exposed to possible amputation and crushing hazards.

29 CFR 1910.215(d)(1): Wheel(s) on grinding machine(s) were not sounded (ring test) and inspected for damage before mounting: On or about June 7, 2016, and at times prior thereto, in the medium and heavy shops employees were not instructed how to conduct a "ring test" to check for cracks prior to mounting a new wheel on a grinding machine. Employees were exposed to flying fragments in the event of exploding abrasive wheel.
