29 CFR 1910.146(d)(3)(iii): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not develop and implement the means, procedures, and practices necessary for safe permit space entry operations, including isolating the permit space.  a)    On or about 2/13/2020, The company completed a confined space entry permit for #3 Dryer on TM-5 which did not include blocking or disconnecting all mechanical linkages which would have kept the dryer from rotating when the dryer was entered. Early on 2/14/2020 an employee entered the dryer to inspect the repairs that had been made and when it started to rotate his head was pinched between a dryer spoke and a stationary element outside of the dryer.
