29 CFR 1910.1048(i)(3): When there was a possibility that an employee's eyes could be splashed with solutions containing 0.1 percent or greater formaldehyde, the employer did not provide acceptable eyewash facilities within the immediate work area for emergency use.      a.) Allegheny Store Fixtures, Inc. DBA Allegheny Trico - On or about June 9, 2016, and times prior thereto: employees were exposed to paints containing formaldehyde at concentrations greater than 0.1 percent. Facilities capable of removing formaldehyde from the eyes were not provided.

A machine operator was exposed to a mixed noise level of 88.5 dBA or an equivalent dose of 82.14%, as an 8-hour TWA, while conducting cutting operations. This level was based on 451 minutes of sampling; zero exposure was considered for the unsampled time.
