29 CFR 1910.269(a)(2)(i)(A): Each employee was not trained in, and familiar with, the safety-related work practices, safety procedures, and other safety requirements in this section that pertain to his or her job assignments.   (a)  On or about May 24, 2016 at the 200 N. Grand Ave, Rothschild, WI jobsite; employees working in the High Voltage Switch Yard Substation were not trained and familiar with the safety-related work practices, safety procedures, and other safety requirements when removing stubbed wires for an Allis-Chalmers switchgear.  Employees were exposed to electrical shock hazard from an energized 4160 VAC lightning arrester.

29 CFR 1910.269(a)(3)(i)(A): The contract employer did not ensure that each of its employees were instructed in the hazardous conditions relevant to the employee's work that the contract employer was aware of as a result of information communicated to the contract employer by the host employer under paragraph (a)(3)(i) of this section:   (a)  On or about May 24, 2016 at the 200 N. Grand Ave, Rothschild, WI jobsite; the employer failed to identify all hazards employees would be exposed to when working on the Allis-Chalmers (S.O. #21-1800-2722) switchgear located in the F-2 cubicle.

Grand Ave, Rothschild, WI jobsite; the employer during the Pre-Task Planning did not determine the location of the 4160V AC lightning arresters and grounding cable to be hazardous exposing employees to electrical hazards.
