Grand Ave, Rothschild, WI jobsite; employees working in the High Voltage Switch Yard Substation were not trained and familiar with the safety-related work practices, safety procedures, and other safety requirements when removing stubbed wires for an Allis-Chalmers switchgear. Employees were exposed to electrical shock hazard from an energized 4160 VAC lightning arrester.

Grand Ave, Rothschild, WI jobsite; employees working in the High Voltage Switch Yard Substation were not wearing appropriate personal protective equipment when working in close proximity to an energized lightning arrester in the Allis-Chalmers Shelter-Clad F-2 cubicle switchgear. The 4160 VAC lightning arresters were not tested and verified as de-energized prior to performing work exposing employees to electrical hazards.
