29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. (a) On or about June 13, 2016, and at times prior thereto, subcontractor employees were exposed to fall hazards greater than 8 feet while installing electrical equipment next to wall openings, where no guardrail was installed or other fall protection available or used. (b) On or about June 13, 2016, and at times prior thereto, general contractor employees were exposed to fall hazards greater than 8 feet, while pumping water out of the basement sump pit, where no guardrail was installed or other fall protection available or used. (c) On or about June 13, 2016, and at times prior thereto, a subcontractor employee was exposed to fall hazards greater than 17 feet, while standing at an exterior and unguarded wall opening, where no guardrail was installed or other fall protection available or used. NOTE: Abatement certification and documentation is required for this item.

(b) On or about June 13, 2016, and at times prior thereto, general contractor and subcontractor employees were exposed to fall and trip hazards when ascending and descending the first floor to the second floor mezzanine, where no handrail was installed along the stairs.
