29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: a) On or about September 21, 2017 and at times prior thereto; employees were exposed to a fall to concrete ground from a height of approximately 20 feet while operating an aerial lift to access the roof of a metal building greater than 20 feet without the protection of a safety harness and lanyard. Note* Abatement certification IS required for this item.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems. a) On or about September 21, 2017 and at times prior thereto; in windy conditions, employees were exposed to fall hazards fall hazards greater than 20 feet while sheeting the roof of a metal building and where no guardrail was installed or other fall protection was available. Note* Abatement certification AND documentation IS required for this item. The Bar S U Inc./DBA Schmeeckle Brothers Construction Co. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1), which was contained in OSHA inspection number 1155282, citation number 1, item number 1 and was affirmed as a final order on 9/20/2016, with respect to a workplace located at 17365 South Cole Road, Kuna, Idaho 83634.
