29 CFR 1910.23(a)(8)(ii): Every floor hole into which persons could accidently walk was not guarded by a floor hole cover of standard strength and construction: On or about June 8, 2016, at the AAFES Troop Store loading dock, employees were unloading merchandise next to an unguarded hole measuring eight inches wide and nearly three feet long created between the loading dock and the tractor trailer exposing employees to injury.
