29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:    On or about June 16, 2016, two employees were cutting materials on the second floor of the residential home under construction without wearing protective headgear. The employees were handing and throwing the materials to two other employees working on the roof, exposing the employees to the hazard of being struck by falling objects.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  On or about June 16, 2016, four employees were conducting framing operations on a residential home under construction without the employer providing training on the potential hazards on the site. Employees were using pneumatic nail guns and circular saws without eye protection, and working within the structure without head protection where overhead hazards were present. The employees were exposed to the hazard of being struck by falling and flying objects.
