29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    On or about 24 June 2016 and at times prior thereto, two employees without head protection were performing framing activities on a two story multi-family residence under construction, exposing the employees to the hazard of being struck by flying or falling objects.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents    On or about 12 July 2016, employees without eye protection were performing framing activities on a two story multi-family residence under construction, exposing the employees to the hazard of being struck in the eyes by flying objects.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).    On or about 24 June 2016 and 12 July 2016, employees without fall protection were performing framing activities on a two story multi-family residence under construction, exposing the employees to the hazard of falling approximately 20 feet to the ground below.  A) On 24 June 2016, five employees were working without fall protection on the second story walls, approximately 10 feet above the ground, and one employee was working from the second story top cap, approximately 20 feet above the ground.  B) On 12 July 2016, one employee was working on the second story roof cornice, approximately 20 feet above the ground, and another employee was working on the front porch roof, approximately 10 feet above the ground, both without fall protection.
