29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders shall be used at an angle such that the horizontal distance from the top support to the foot of the ladder is approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support). a. On or before June 16, 2016 where an employee was working from an aluminum extension ladder, the footing of the ladder was at an angle exceeding the 4 to 1 ratio, thereby exposing the employee to a 7 foot fall hazard.

29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee shall be protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. a. On or before June 16, 2016 where employees were installing vinyl siding to a three story building with pump jack scaffolds, the employees were not protected from falls, thereby exposing the employees to a fall hazards of up to 20 feet. Manuel Ruiz was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 190133, citation number 1, item number 2 and was affirmed as a final order on February 15, 2012, with respect to a workplace located at 8321 Parori Lane, Blacklick, OH 43004.
