29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: On or about 6/20/2016 at 2289 East Rock Quarry Road, Buford, GA: Attachments used on the forks of the powered industrial trucks to elevated employees to heights up to 15 feet and to lift and move large recreational vehicles were not approved by the manufacturer prior to use. a. Metal elevated personnel platform basket was used to elevate employees to heights up to 15 feet from ground level in order for them to clean the tops of the recreational vehicles. b. Fabricated metal bracket attached to the forks of the powered industrial trucks was used to lift the recreational vehicles and move them throughout the parking and maintenance areas.

29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): On or about 6/20/2016 at 2289 East Rock Quarry Road, Buford, GA: Where employees operated a powered industrial truck to lift recreational vehicles, employees on a metal platform, and other material with defects such as but not limited to hydraulic leaks, non-operational lights, and missing and loose back rest bolts. The employer failed to remove the powered industrial truck from service until the repairs could be made.
