29 CFR 1910.133(a)(1): Protective eye equipment was not required where there was a reasonable probability of injury that could be prevented by such equipment:    On or about June 28, 2016:  Employees are exposed to chemical splash hazards.  a.  In the tank fill area where employees fill propane tanks.  b.  In the vehicle wash area where employees mix chemicals such as but not limited to bleach and TNT wash which is a corrosive.

29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use.   On or about June 28, 2016: The spray painter who is required to use a Tsunami SAS Supplied Air Respirator is exposed to inhalation hazards from chemicals such as but not limited to hexamethylene diisocyanate.  The employer did not have a written respiratory protection program.

29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace.   On or about June 28, 2016: The spray painter who is required to use a Tsunami SAS Supplied Air Respirator is exposed to inhalation hazards from chemicals such as but not limited to hexamethylene diisocyanate.  The employer did not ensure that the employee was medically able to wear a respirator prior to initial use.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use.   On or about June 28, 2016: Employees working in the RV wash area are exposed to ZepInc Truck & Trailer Cleaner which is a corrosive.  The employer did not have a means for flushing of the eyes in the RV wash area.

On or about June 28, 2016: Employees working at the facility located at 2289 E. Rock Quarry Road are exposed to chemicals such as but not limited to Zep Tire Shine, motor oil, brake cleaner, liquid petroleum, degreasers, Clorox Bleach, antifreeze and ZepInc Truck & Trailer Wash which is a corrosive.  The employer did not have a written hazard communication program.

On or about June 28, 2016: Employees working at the facility located at 2289 E.
